Insurance

Seven Hills Surgical Associates participates with the following insurance plans:

  • Medicare and all Medicare managed care plans
  • Medicaid and all Medicaid Managed Care plans
  • Anthem BlueCross and BlueShield PAR and PPO (not HMO)
  • Southern Health
  • Piedmont Community Health
  • CIGNA
  • Tricare

At this time, we do not participate with Aetna or United Healthcare.

We will file charges for office visits and surgeries to insurance carriers, even if our surgeons are not participating providers.  We will also file claims to secondary insurance carriers.  Co-payments, deductibles and co-insurance are part of the contract between the insurance company and this office and will be collected.  Your insurance policy is a contract between you and your insurance company.  We are not a party to that contract and payment of your bill remains your responsibility.

If you do not have your insurance card with you at the time of your visit, you will be expected to pay for your visit at the time of service.  Please remember that it is your responsibility to keep us informed of any change in insurance carriers or insurance status.

Referrals and Co-pays

Many insurance providers require a referral from your primary care physician.  Services rendered without this referral may not be covered and will be billed to you.  It is the patient’s responsibility to obtain this referral prior to your appointment.  Your plan may also require a co-pay for office visits and procedures.  Please be prepared to pay this at the time of your appointment.

Surgical Pre-certifications

If surgery is required and insurance pre-certification is needed, our office will attempt to obtain that pre-certification.  If we are unable to obtain that pre-certification, you will be notified prior to surgery so you may attempt to obtain it.

If you are uninsured and having non-urgent or elective surgery, we may require partial payment before the date of the surgery.  If you need urgent or emergency care, we will not delay your surgery.

Workers’ Compensation

If you have a workers’ compensation injury, you must contact your employer, workers’ compensation insurance company and be assigned a case worker and claim number before we are able to treat you.  WE WILL NOT MAKE AN APPOINTMENT WITHOUT THIS INFORMATION.

Self Pay

If you have no insurance, we will discuss the financial aspects of your care.  Payment is due at the time of service unless prior arrangements have been made.  We also offer a self pay program for those who qualify.  Based on our review of your most recent tax return, your fees may be reduced according to our program guidelines.  We will then set you up on a monthly payment plan.

Payment Options and Billing Questions

We accept payment of fees by cash, check, VISA or MasterCard.  Monthly payment plans are also available.  For billing questions or credit card payments, please call our business office at (434) 947-5030.  Payments may be mailed to Seven Hills Surgical Associates, PO Box 11766, Lynchburg, VA 24506-1766.
